An FMCSA-listed medical examiner reviews your health history and assesses whether you meet the physical qualification standards for commercial driving. The visit includes vision, hearing, blood pressure and a physical examination, along with the testing required for the medical examination. Tell the examiner about medicines, prior surgery, sleep apnea, diabetes, heart conditions and any other relevant history. The examiner determines whether you qualify and for how long. A certificate may be valid for up to 24 months, but some medical conditions call for a shorter interval or more information. The exam is an evaluation, not a guarantee of certification. A DOT physical is different from a drug test; ask separately if your employer requires drug screening.
Bring your driver’s license or other requested identification, a list of current medications and any glasses, contacts or hearing aids you use. If you have a condition that may affect certification, ask whether to bring a recent note, test result or treatment record from your clinician. This can help the examiner make a timely decision. Give your name, date of birth and license details exactly as they appear on your license when completing the driver form.

The DOT physical costs $105. Ask the team whether any separate service you request has an additional charge. After the exam, the medical examiner explains the determination and the next steps for your certificate and driver records. For CDL and commercial learner’s permit holders, medical certification information is transmitted through FMCSA and state licensing systems; ask how to check that your record updates. Keep any copy provided for your records.
